ELIGIBILITY REQUIREMENTS:


  1. 1. Anyone from the black community can register.
  1. 2. The candidate must be a Quebec resident during the census period, for the project being nominated.
  1. 3. The projects submitted must have been marketed (a strategy put in place to publicize and promote the projects or achievements) or have had a significant impact.


One form may be submitted per category, up to a maximum of three (3) categories per entrant. Only one (1) project may be submitted per category. Additional forms will not be considered.


Applications may be submitted by the candidate or by an authorized third party.


Incomplete or unauthorized applications will not be considered. An e-mail confirming receipt of the application will be sent to the address to be contacted when the form is submitted.

The text of your biography must be written in the third person. This text will be used to present you on our various platforms if your application is selected as one of the finalists.
12) Official photo
Please upload a recent, professional photo of the candidate, respecting the following specifications: 300 dpi, jpeg format, portrait and upper body. Horizontal (landscape) orientation is preferred. The submitted photo will be used for promotion and nomination purposes. Maximum image size = 10MB.
